Annually from the date of enrollment
When a driver has any of the following actions/activities added to his/her driver record:

Any other actions taken against the driving privilege
Any employee who is required to have a California Driver’s License as a condition of their employment must complete a DMV Authorization for Release of Driver Record Information INF 1101 form.
